All forms for participation in the Resource Mobilization Plan (RMP) are available in an electronic format or a hardcopy (pdf) format. The forms include the determination for reimbursement rates for participating fire departments. Please save the electronic form to your computer before filling it out. Print the electronic form once it is filled out, obtain signatures, and mail the form to the proper district office.
FIRE DEPARTMENTS ARE RESPONSIBLE FOR USING THE CORRECT REIMBURSEMENT RATES FOR THEIR DEPARTMENT, EQUIPMENT TYPE, AND PERSONNEL FITNESS AND TRAINING WHEN SUBMITTING FORMS FOR REIMBURSEMENT.
